Complete Intake Packet
To register with Student Disability Services, and receive accommodations, you must be a new or current student and have or suspect you have a disability. Students must complete an intake packet prior to attending the intake meeting. All student information is maintained securely and confidentially.
Schedule Intake Appointment
Students must schedule an intake appointment with the Student Disability Services assistant director at the campus they are attending. Intake appointments last approximately one (1) hour. Potential students are recommended to contact the office at least one semester prior to attending. This allows time for a review of documentation, and prevents a delay in the provision of certain accommodations. Parents may attend the initial visit, with the student's permission.
Provide Documentation
Please bring documentation you may have supporting your disability with you to your intake meeting, along with your completed intake packet. Documentation may be a recent high school IEP, 504, medical or psychological documentation. Documentation should provide an adequate representation of the student's current functional abilities. Additional ways to secure documentation may be discussed during your initial intake meeting.
Accommodation Notification
Once approved, you will need to provide a copy of your accommodations you wish to utilize to each of your class instructors. To understand accommodations that are available and procedures please continue with this website.
