    Testing Referral Form for Testing with Accommodations

    The Online Testing Referral form was developed to allow faculty to send tests and test instructions quickly and conveniently. Please find the Testing Referral form at: 

    MyUNG > Forms &Requests > Student Accessibility Services > Testing Referral Form. (Or on the Student Accessibility Services website under Faculty and Staff Information)

    The Testing Referral form is completed as follows:

    • Regular Time Allotted: the maximum amount of time that the class as a whole will be allowed
    • Materials/Aids Allowed: for example, a calculator, text book, and/or scratch paper
    • Special Instructions: additional information or special concerns that the test proctor needs to know, such as issues pertaining to designated time frames
    • Test to be Administered On: Date the class is taking the test
    • Or Test to be Completed By: Date that the student must have the test completed
    • After Test is Completed: Select whether you would like to have a) the test to be returned by interoffice mail, or b) the test to be held for you to pick up Returning tests through campus mail is secure and reliable. If delivery schedules are a concern, and “Will Pick Up” is selected, the test will be retained in a locked cabinet until you or a designee pick it up.
    • Click to attach file (box on lower left): Enter the URL for the test to make it immediately available to testing personnel

    Testing Referral forms must be submitted in advance. For questions about test submission time frames, contact campus Testing staff. For questions about student accommodations, contact Student Accessibility Services.
