Machform is an easy to use, standalone form software with a drag-n-drop interface. It allows you to create online forms quickly that you can link to or embed in a web page. Data is collected in spreadsheet form and you can receive email notifications upon submission.
For more advanced users, you can create conditional statements to show/hide fields or pages based on selections.
