Frog Form

1. Download the Cadet Application and Enrollment Record Form (PDF)
2. Double click on the form to open it. 
   • It should open in a new window.
3. Wait for instructions on when and how to fill out the form. 
   • Do not go beyond the steps for which were instructed
4. Once instructed, select the print icon in the top right.
5. Do not save the form, but do not close the form until told to.
6. Once informed it is okay to close the page, close it without saving the form.
   • This is to protect your data and privacy.
